COMMUNITY PERISH NURSE PROGRAM
Parish nursing services are designed
to involve individuals, families and congregations as active partners in their personal health. Through this involvement,
nurses help empower communities and foster an environment of understanding and support. Regardless of faith or religious affiliation,
any registered nurse that is a member of a congregation is invited to be a parish nurse. With knowledge of faith and healthcare,
the parish nurse can clarify issues and reinforce the strong tie between faith and health. A parish nurses can provide individual

How to get medication resources via
Internet. Many drug manufacturers have Patient Assistance Programs. These programs are designed to help those who cannot afford
their medications -- many can be obtained at no cost or low cost. These web sites provide free information and application
forms. 1) www.needymeds.com - this is the most comprehensive site for assistance. You might suggest the caller start with this web
site. 2) www.TogetherRxAccess.com - ten major drug companies created this program to help qualified individuals and families who lack prescription
drug insurance to save on brand-name prescription drugs, other prescription products, as well as a wide range of generic drugs.
There are five pharmacies in Blm-Normal that have agreed to accept the card. If a person does not have Internet access, they

no patient in need of their medicines will go without them. HelpingPatients.org also contains a database of all the member
companies that conduct these programs and the medicines that are covered. It also describes how to request assistance. The
application process and eligibility requirements vary for each company and program. Medicines available through the programs
include many of the newest and most innovative brand-name treatments available, including medicines that treat: cancer, high
cholesterol, diabetes, high blood pressure, stroke, depression, schizophrenia, and Alzheimer's.

CARD PROGRAM FOR THE UNINSURED - Ten major drug companies recently got together and rolled out a new discount drug program
for uninsured Americans. Together Rx Access™ has been created to help qualified individuals and families, who are not
covered by prescription drug insurance, to save on brand-name prescription drugs, other prescription products, as well as
a wide range of generic drugs. Applicants must meet all of the following criteria to be eligible for this program: a) not
eligible for Medicare b) no prescription drug coverage (public or private) c) legal US resident d) household income equal
to less than $30,000 for single person; $40,000 for family of 2; $50,000 for family of 3; $60,000 for family of 4; $70,000
for family of 5. For families of 6 or more people, contact Together Rx Access at 1-800-444-4106 to determine eligibility.
There are no enrollment fees, no monthly fees, and no hidden fees. A two-page enrollment form can be completed on the web

medications used for the treatment of heart and blood pressure problems, diabetes (including insulin, syringes, and needles
used to administer insulin), arthritis, Alzheimer's disease, cancer, lung disease and smoking related illnesses, osteoporosis,
glaucoma, multiple sclerosis, and Parkinson's disease.
$4.00 MEDICATIONS
The generic prescriptions available
at Target, Walmart, and Meijer include psychiatric medication, but there are many other types of meds available as well including
blood pressure and heart medication. You can get a list online for Walmart www.walmart.com, or Target www.target.com, or ask the pharmacist at these stores. Meijer's and Walgreen's programs seem to not be online, but you
can telephone their pharmacists. Also, Meijer has FREE antibiotics available.

Ministering to the Needs of 8,000 families
in Vermilion, Ford and Iroquois Counties each year. Weatherize homes for homeowners with qualifying income levels at no charge.
Renters may be eligible for services if their landlord agrees to pay one-half of the cost of materials and labor applied to
their residence.
Qualifying families receive a payment
on their behalf directly to their energy provider to defray a portion of their winter heating costs. The amount of assistance
each family receives is dependent upon income level, household size, and type of fuel used. Senior citizens, the disabled,
and households who are disconnected from their power source may apply during a special early application period in September
and October.

Designed
to prevent and reduce student problems of substance abuse, teen pregnancy, suicide, truancy, dropout, violence, child abuse,
behavior problems, and other serious life issues. The SAP team is made possible through the efforts of school administrators,
SAP coordinators and counselors, school psychologists, or other mental health professionals, teachers, coaches, nurses, students,
and community representatives. The most successful SAP teams work closely with a well defined community network of treatment
programs, social service departments, and law enforcement agencies.Reported benefits of SAP teams include decreased vandalism,
increased student participation in alcohol and drug-free functions, more positive attitudes toward youth with problems and
improved communication between students, staff, community and parents. An additional benefit is a decreased dropout rate for
students with problems. The goal of the SAP team is to identify adolescents whose health, behavior, attendance or grades indicate
that they are at risk. Through the different steps that comprise the SAP process, these at-risk adolescents are directed to
appropriate services in the school and community. The services will provide the necessary assistance to help students deal
with his/her problems. To make a referral to the SAP team, parents may contact a team member. Students may also refer a peer
by contacting a SAP member verbally or in writing.
